New Patients

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. These are available from our reception.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Register Your New Baby

If you are the proud parent of a new baby, you may not be thinking about the need to register him/her with a doctor, but it is in the baby’s best interest to do this as soon as you can. There are forms at the surgery that you can use to register your baby.

When the surgery is informed of your baby’s birth, we will send Mum an eight week post natal appointment and your baby’s first immunisation appointment.

Named GP for All Patients

INFORMATION FOR PATIENTS

All practices are now required to provide their patients (including children) with the name of their named GP who will have overall responsibility for the care and support that the surgery provides to them. This is an administrative exercise that will make no difference to the way you are cared for at the practice. Patients at Moreton Cross Group Practice have always been allocated a named GP on registration.

What does ‘accountable’ mean?

The new contract requires the named accountable GP to be responsible for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each patient where required. However, this does not mean that they will be the only GP or clinician who will provide care to that patient. These responsibilities will be carried out within the opening hours of the Practice and do not change the way you currently access care outside these hours.

Will GP practices write to patients to inform them of their named GP?

No. However, practices are required to inform patients of their named GP at the next appropriate interaction.

Can patients choose their own named GP?

Patients will be allocated a named GP, however if a patient requests a different named GP, reasonable effort will be made to accommodate their preference.

Do patients have to see their named GP when they book an appointment?

No. Patients can, and should, feel free to choose to see any GP or nurse in the practice in line with current arrangements. If their preferred choice of GP or nurse is not available, an alternative will be offered. As all patients have an electronic medical record this ensures that all clinicians in the Practice have access to the most accurate and up to date information.
