Register with our practice
If you live within our practice area you are welcome to register with us.
You can now register with us online via the following link https://gp-registration.nhs.uk/N85028/gpregistration/landing
or by scanning the QR code below.
If you cannot register online you can print off and complete form PRF1 which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. These are also available from our reception.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Register Your New Baby
If you are the proud parent of a new baby, you may not be thinking about the need to register them with a doctor, but it is in the baby’s best interest to do this as soon as you can. There are forms at the surgery that you can use to register your baby.
When the surgery is informed of your baby’s birth, we will send Mum an eight week post natal appointment and your baby’s first immunisation appointment.
Catchment Area (Practice Boundary)
You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.Named GP
All practices are now required to provide their patients (including children) with the name of their named GP who will have overall responsibility for the care and support that the surgery provides to them. This is an administrative exercise that will make no difference to the way you are cared for at the practice.
Accessing someone else's information
From 31st October 2023, you will start to see new information from your doctor (GP) on your online account.
You can see this through the NHS App, website or another secure record access app. This information will include notes from your appointment as well as test results and any letters about future appointments.
